MASHPEE – The Community Health Center of Cape Cod has received a $10,000 grant from the Cape and Islands United Way.
The Community Impact Partner Grant will support the Health Center’s Complex Care Needs Program which helps patients who may have one or more chronic health conditions expected to last a year or longer.
The care team works with primary care providers to develop wellness plans that may incorporate dietary changes, exercise, education and guidance.
The non-profit health center provides access to health care for adults and children regardless of insurance status or ability to pay. The center operates out of locations in Mashpee, Falmouth and Bourne.
